  • Be treated by a healthcare provider licensed in an outpatient setting in the United States, Puerto Rico, or US Virgin Islands.
  • Live in the United States, Puerto Rico, or US Virgin Islands.

The difference in the program for uninsured patients vs. underinsured patients is that uninsured patients are enrolled for 12 months but patients with insurance are enrolled through the end of a calendar year, since many people switch plans at the end of the calendar year.
